The State of Maryland established a State Coordinating Council (SCC) and a Local Coordinating Council (LCC) in each Maryland jurisdiction. The SCCs and LCCs are charged with reviewing cases of youth placed into in-state residential placements and out of state placements to ensure that these placements are necessary, and that there are no other available resources which would allow youth to remain in their homes and communities during treatment. The LCCs also have access to interagency funding, which can be used to return or divert youth from instate residential and out of state placements. The State is committed to serving youth in the least restrictive and most appropriate environments possible, and the SCC and LCCs collaborate with State and local agencies to accomplish this.
The Queen Anne’s County Local Coordinating Council (LCC) ensures that children and their families, with special needs and may need to be placed outside of the home receive the necessary supports and resources to live a successful life. The LCC reviews the situation of each child and family referred and their plan of care to meet the child’s needs. When all community resources have been exhausted and/or it is found to be in the best interests of the child, the LCC will develop and implement an inter-agency plan of care for children in need of residential placement. The LCC will then follow the progress of the child and family and modify the plan as necessary.

RTC Waiver Program.
The RTC Waiver is a program that uses a team-based approach called Wraparound to provide mental health and medical services to youth with intensive mental health needs. The team, including the youth, family members, care coordinators and service providers, develops a plan of care that allows the youth to receive services and supports in the community rather than in a residential treatment center (RTC). The plan of care and services are individualized to the strengths and needs of each family. Youth can receive services for up to two years at no cost to you.
Services are provided through Wraparound Maryland. Their goal is to provide highly responsive, family driven, youth guided services that are unique to every family. Their approach is team based and focuses on the strengths of each member of the family and of the team. It is important to them that each family has a voice and feels empowered to make decisions for their family, as they are the experts of their family.

Steps to make a referral to Queen Anne’s County Local Coordinating Council - Explains all the steps that should be taken to make a referral to LCC.
LCC - Release of Information Form - Form to be signed by the caretaker of a child in order to be allowed to have their case heard at LCC meeting.
LCC/SCC Referral Packet - Provides all the most up to date documents needed in order to make a referral to LCC or submit an application to the SCC for review. Please note that the forms must be typed. When finished with the packet save to your computer print and send via email to Cindi Bauer (cbauer@qac.org). The packet includes: release of Information, LCC/SCC Referral Form, LCC Referral Packet Checklist, SCC Referral Packet Check List, 10 Day Waiver and sections 1- 15.
Vendor Approval for Services - Document to be signed by the service provider acknowledging that a contract is forthcoming for the identified child and or family receiving services through care coordination services. Additionally it lists whether or not the vendor is bound by Maryland background check requirements or not.
Required Queen Anne's County Vendor Forms: Vendors to the Queen Anne's County Community Partnerships for Children and Families are required to fill out a W-9 form in its entirety and complete a separate County agreement form (both forms are combined when you click the link). These forms must be on file in the County Finance office (Accounting Division) before any checks will be issued. Once the forms are complete, fax them along with your first invoice to: (410) 758-6904.

LCC FORMS
The State Children Youth and Family Information System (SCYFIS) is the Web Based case management program utilized for tracking children and families reviewed by the LCC. All members of the LCC are allowed to have access. The program may be accessed by clicking here.
